The partnership came about naturally, with TrilogyCare seeing the same gaps in regional aged care services that we are so familiar with. While major providers focus on high-volume urban markets, rural communities often struggle to access quality home care services that understand their unique needs and contexts.
Trilogy Care brings specialised expertise in aged care navigation and package funding management, while we contribute our regional service delivery experience to provide Care Coordination and in-home supports. Together, we can offer comprehensive support that addresses both the practical and emotional needs of older people choosing to age in place.
What This Partnership Means

One of the biggest challenges we've observed is the cliff-edge between NDIS and aged care services. Someone might receive excellent disability support through the NDIS until age 65, then face a completely different system with different providers, approaches, and philosophies once they hit the traditional cut-off age for NDIS support.
Our partnership with Trilogy Care creates pathways for continuity. We can maintain relationships and understanding of individual needs while navigating the transition between funding systems. This is particularly important for neurodivergent older people who may struggle with change and unfamiliar providers.

Family carers in regional areas often carry enormous responsibility with limited formal support. Both organisations prioritise carer wellbeing and sustainability, recognising that supporting the carer often enables the older person to remain at home longer and more successfully.
The partnership will initially focus on the Central Coast and lower Hunter regional and rural regions, with plans to expand into other rural areas of NSW based on demand and community need. We're particularly excited about reaching communities that have been underserved by mainstream aged care providers.
